Once notified of a member’s death we will write to confirm the information we need to determine the benefits payable, this will include:

  • Deceased Member Information Form
  • Death Certificate
  • Marriage Certificate
  • Birth Certificate (adult dependant)
  • Birth Certificate (eligible children
  • Copy of Will
